The Bellevue Hospital's Family Health Services, located at 402 W. McPherson Hwy. in Clyde, now offers sports physicals for athletes.
Sports physicals are open to the public. Patients do not have to be previously established with Family Health Services to have a physical performed.
A $30 cash only fee will be charged for sports physicals. There is no insurance billing available for the sports physicals so the fee must be paid at the time of the visit. Patients are responsible for bringing their own sports forms and documents with them at the time of the visit. If a patient plays multiple sports and has their forms with them at the appointment, one additional sports form may be completed and included in the $30 fee.
Appointments for sports physicals may be made:
Mondays & Tuesdays - 8:00 a.m. - 7:30 p.m.
Wednesday & Thursdays - 8:00 a.m. - 4:30 p.m.
Fridays - 8:00 a.m. - Noon
Walk-ins may be accepted but appointments are preferred.
